Basic overview
Long term conditions include:
Diabetes, asthma, COPD, coronary heart disease (CHD), hypertension, hypothyroidism, heart failure, kidney disease, rheumatoid arthritis, stroke/TIA, pre-diabetes & previous history of gestational diabetes.
You will be invited to make an appointment for your annual long term condition review around your month of birth.
We hope that by doing this, the nurse/health care assistant can review all your conditions in one appointment to save you having to attend numerous appointments throughout the year & having multiple blood tests. (From time to time, the nurse/hca may request to see you for a follow-up appointment at their discretion).
We will send you the initial invitation either by SMS or post, which is then usually followed by a reminder 2-3 weeks later.
If you make an appointment but then still receive the reminder, please do not worry as this is only a prompt & you should still attend the original appointment as arranged.

- Patients diagnosed with asthma and/or COPD ONLY (no other conditions) will be asked to make a telephone appointment with a practice nurse or in-house pharmacist.
- Patients diagnosed with pre-diabetes, hypothyroidism or who have a history of gestational diabetes only will not be asked to make an appointment. This review requires an up-to-date blood test only. We will only be in-touch following these results should a Clinician wish to speak with you.
- Patients diagnosed with Kidney Disease and/or Diabetes will need to provide a urine sample. To do so, you will need to collect a thin, white, test-tube sized bottle from the main reception at the Health Centre and attach a label which we will provide. Any unlabelled samples will not be able to be tested. It is best if the sample can be taken from the first stream of the day and needs to be placed in the provided pathology form and handed in to either ourselves, or the path lab, Goole Hospital.
- Patients diagnosed with all other conditions will need a face-to-face appointment with either a practice nurse or hca. This will be specified in your initial invitation as to who to make the appointment with.
- Patients with multiple conditions will only need one appointment, although more time should be allowed for this to ensure all conditions are reviewed.
Your long term condition review is a routine appointment and can be pre-booked in advance. We suggest that you contact the surgery in an afternoon as the phone lines may be quieter, although we cannot guarantee this.
Bloods
You will be advised in your text/letter if we need you to have your bloods taken prior to your long term condition review appointment.
You must now ring Goole and District Hospitals’ phlebotomy appointment line on 03033 306670 to arrange this. Phone lines are open Monday to Friday, 9.00am to 4.00pm.
To ensure that the nurse/hca has the blood results for your review, please have these done at least one week before.
If you have received your invitation by text, then your pathology form will be ready to collect from the main reception at the Health Centre the following business day.
The pathology form will be included in our postal invitations.
